Dear Patient,

Welcome to PAIN SPECIALISTS OF TEXAS! 
We look forward to serving your health care needs.  Enclosed you will find our new patient packet.  Please fill out these forms and either mail, fax, or drop them off prior to your first visit.  By providing us with your information prior to the visit, we can verify all benefits and assure that referrals are in place if necessary.  We value your time and do not want to delay your appointment due to verification of information. 

If you choose to fax back your paperwork, please bring the originals on the day of your appointment.  We will keep them in your file.

Our office is opened Monday-Thursday from 7:30 am-4:30 pm.  The last appointment of the day is scheduled at 4:30 pm.  The office is closed from 12:30-1:30 pm for lunch.  Fridays, the office is closed due to the physicians doing procedures at other sites.

If you need immediate assistance when our office is closed,  we urge you to go to the nearest emergency room or contact your primary doctor.  The facility you choose will contact either physician if necessary.

All prescription requests must be filled during normal business hours.  No exceptions are made.  Samples are not kept at the office for any medication.  Please allow 72 hours for your normal refills.  Your pharmacist can either contact us at (972) 606-8980 or fax the request to (972) 606-8923.

Please bring a list of all your medication (prescribed, over-the-counter (OTC), and supplements/herbals) to each visit so that we may update your chart.

If a referral is required for your appointment, please contact your primary care physician to ensure the referral is in place for your visit.  Without a valid referral on file, we will give you the option of paying for the visit or rescheduling until your primary care physician can obtain one.
